I certify that everything that I have stated on this application is true to the best of my knowledge. I authorize you to check my submitted paperwork and confirm company information. If I change service address or add an additional service address, I will be bound by this application. I understand that non-payment of my account will result in discontinuation of service.

Upon acceptance of this Application by the South Georgia Governmental Services Authority, d/b/a CNSNext:

Customer shall be entitled to terminate CNSNext services at any time, and CNSNext shall be entitled to terminate its services at any time. Upon the termination of services all CNSNext equipment shall be returned to CNSNext.

CNSNext will bill for services at its current rates, as adjusted from time to time. Payment is due within (18) days of the date of the invoice.

CNSNext will use its best efforts to provide the services identified above and to restore service as soon as possible following any interruption. CNSNext does not warrant that services will be uninterrupted, error-free, or free of latency or delay, that the services will meet the customer’s requirements, or that the services will prevent unauthorized access by third parties. CNSNext is not responsible for a customer’s loss of data, and customer is advised to back up all data, files and software prior to the installation of service and at regular intervals thereafter.

CNSNext MAKES NO WARRANTY, WHETHER EXPRESS OR IMPLIED, WITH RESPECT TO ITS SERVICES AND EXPRESSLY DISCLAIMS ANY WARRANTY OF MERCHANTABILITY OR WARRANTY OF FITNESS FOR A PARTICULAR PURPOSE. IN NO EVENT SHALL CNSNext BE LIABLE FOR SPECIAL, INCIDENTAL, OR CONSEQUENTIAL DAMAGES, OR FOR LOST REVENUES, PROFITS, OR GOODWILL OF ANY KIND.

Application for service will not be accepted until the applicant, or the person for whom the application is being submitted, has paid or made satisfactory arrangements to pay CNSNext all sums then owed and unpaid for services previously rendered by CNSNext, whether at the current location or any other premises. After service has been established, CNSNext reserves the right to discontinue service if it finds the service is being shared with, or is fully or partially benefiting, a person who owes CNSNext for past service, regardless of location.

Customer is responsible for any loss or damage to CNSNext-owned equipment caused by negligence, misuse, or the actions of household members, guests, children, or pets. In such cases, customer agrees to pay the full replacement cost as determined by CNSNext.

I agree to pay any and all collection costs that may be incurred by a third-party vendor to facilitate the recovery process of my outstanding amount due.
